SMART health card verification app.

There have been 101,199 successful downloads to the Hawai’i SMART Health Card app, since it launched on Friday last week.

The app was launched before Maui’s “Safe Access O’ahu” program and “Safer Outside” program which requires proof of vaccination to be able to dine inside restaurants. Both programs accept the Hawai’i SMART Health Card as proof of vaccination. Obtaining the digital card is voluntary and those who do not have one can still use a hard copy or photo or their vaccination document.

The download rate represents approximately 75% of all attempts to create a SMART Hawai’i health card. The Office of Enterprise Technology Services will continue to analyze the data to see how the pass rate can be improved over time, but ministry officials say the current pass rate is expected.

“I am delighted with the number of participants the Hawai’i SMART Health Card has received since the program launched last week,” said CIO Douglas Murdock, Chief Information Officer, Office of Enterprise Technology Services, State of Hawaii ‘i.

Ministry officials say the success of generating a SMART health card depends on the accuracy of the data entered by the individual and the provider into the database. Entering human data from variations of names entered in the data could be an example of why a vaccine document cannot be verified automatically by Hawai’i SMART Health Card.

If the person received their vaccine from a provider who relates to the database and cannot generate a QR code, they should try again. Here are some helpful tips:

  1. Make sure that the name used in your Safe Travels profile is the same as the one entered in the database. The name in the database comes from the electronic file provided by the service provider administering the shot. So that’s the name you gave the nurse or doctor at the time of the injection. For people who have made an appointment online, please verify the name you entered to make an appointment.
  2. Check your name variations. For example: Doug vs. Douglas, Jones-Smith vs. Jones Smith.
  3. Check the lot number (s) of your vaccine. Make sure it is accurate. For example: 0 is really 0 and not O, and 1 is really 1 and not l
  4. Check your date of birth and date (s) of inoculation. Make sure you entered them accurately.
  5. If you have three injections listed on your vaccination record, indicate the last injection (often listed in the “Other” column) as your final dose.

The Hawai’i SMART Health Card QR code can be added to the Samsung Pay and CommonHealth app now and more apps will be available in the future. We do not recommend that you take a screenshot of the QR code and save it to photos, as it may fail in the verification app. The best option is to show the QR code of the Hawai’i SMART Health Card.

Individuals who received vaccines from entities not submitting data to the state database would not be able to obtain a Hawai’i SMART health card since the program verifies the information entered in the application against the request. database. These entities include: the Department of Defense, the Department of Veterans Affairs, some federal agencies (those who have received a federal vaccine), and some pharmacies under the long-term care facilities program (nursing homes, institutions qualified nursing care, etc.).
