Northwest Health Virtual Care Support Program Chronic Diseases

The program will allow people with hypertension, heart failure, diabetes and other chronic conditions to access health care remotely.

SPRINGDALE, Ark. — Northwest Health announced its new remote monitoring and virtual care program for Northwest Physicians patients managing certain chronic health conditions.

Northwest Health Home Care Management will serve patients with hypertension, heart failure, diabetes and chronic obstructive pulmonary disease. Northwest’s goal for this program is to help patients stay healthy between doctor visits and reduce preventable hospitalizations.

According to Centers for Disease Control and Prevention (CDC)60% of Americans have at least one chronic condition and 40% have two or more chronic conditions.

Through Northwest’s partnership with Cadence, a remote patient monitoring company, qualified patients will be provided with cellular-enabled devices including blood pressure monitors, pulse oximeters, scales or glucometers to capture daily vital information from home. Daily readings are then transferred to Cadence clinicians who can help monitor changes in patients’ condition and provide virtual assistance when needed.

“This new program expands our ability to help patients monitor, prevent and manage chronic disease,” said Scott Musick, DO, primary care physician at Northwest Medical Plaza in Sugar Creek. “Through daily monitoring, we can improve patient outcomes, quality of life, and deploy early interventions when needed.”

Northwest says the benefits of remote patient monitoring can include fewer preventable hospitalizations and reduced cost of care for specific patient populations.
